I have sent him another email to see if the forms he gave me are ok to use before I post them on the net. The sheets are beside my computer and I will put the info out as soon as I hear from him. If you want George as you said I can send you the info to post when the time comes. Thanks guys.

Finally got a response from him he said he is getting an email account just for this purpose as the one he has listed belongs to his wife and it has been over used. He says by the end of the week he will send me the new email info. Soon guys!
