Just bought a 2000 Venetian Red R with 120k. It has been throwing the P0422 code which, from what I hear, could be any number of things. I found out post-purchase the car has an aftermarket 3" DP (& Cat) on it. Could this be the cause? I also noticed that when running, the exhaust temperatures are quite different between the two tips, although there is still airflow from each (one is significantly warmer than the other???). I hear the Cats break down inside after a while and I was thinking maybe something got a bit plugged up and may be causing the code and differing exhaust tip temps. What does everyone think?

Cat efficiency codes on these cars are weird.
Sometimes the reading is false, sometimes not, but with the DP etc who knows!

These cars do not like aftermarket CATS.

Are the 02 sensors OEM VOLVO and how new/old are they?
I have a guaranteed used low mileage Converter I may sell
I had the same code, (2000 V70R with 100K miles), we cleared it.
It came back pretty quickly in about 2 days
I cleared it again.
It has not come back now for 4-5 months and 4000 miles!

Code has been cleared. We shall see how long it lasts! Should I be concerned about the different exhaust tip temps??

Nah, the stock dual exhaust system doesn't have perfect flow balance between the tips, no worries there.

Give me a call if you want to get it up in the air and have a look.
It takes about 5 minutes to put all 4 corners up on jack stands.
Or I may be able to call in a favor and get the car on a lift for inspection.

BTW -, When we were looking at that DP, it looked like someone had
deleted the turbo heat shield. Any input from SS land on lacking a shield?
