If I let my car idle for a few minutes, a substantial amount of bluish grey smoke comes out of the tail pipe when I apply throttle. Any ideas on what it is? It doesn't smoke otherwise.

Blue smoke at long idles = Bad stem seals or (possibly) clogged PCV.
Blue smoke under hard acceleration = Bad turbo.

Again, these are *possible* causes.

At 200k miles, yes, this is a normal expectation.
