Interesting, first failure I have seen that was not directly related to the slave cylinder.
What did the pressure plate side of the disc look like? Picture is only of the flywheel side.
What was the explanation for the failure, it looks like the friction material just pulled away from the rivets. Was this a result of a hard launch at high RPM? Or perhaps just a defective disc?
Did they replace or resurface the flywheel?

No hard launching, it basically just happened on the highway when I wanted to shift from 4th to 5th.
They did resurface the flywheel but they said it wasn't bad at all.
I guess it was a defective disc or maybe the previous owner abused the clutch too much.
Art
This was my situation as well, though mine was on a 5.5k RPM 2-3 hard shift when the pedal went limp. The mechanic said my clutch looked like I'd held a plate at shoulder height and dropped it, just SHATTERED. Apparently the flywheel was so messed up it couldn't be resurfaced, so I needed that replaced as well. Interstingly enough, the slave cylinder was fine...though I had them replace it too while they were in there. All of it expensive ($3K), and of course NONE of it under warranty. If I EVER find the previous owner....
-Steve
